We would like to have this form at home and office visits to keep on file to follow your progress. We want YOU to be successful!

ABOUT YOUR UPCOMING VISIT

Preparation for your appointment

Please arrive 15-30 minutes early for your visit, so that we can take care of all paperwork. During your first visit, we will go over the registration forms filled out by you or your family member. We will then assess any issues that you may have and will also proceed forward with a breastfeeding session while you are in the office. We will work hands-on and allow time for you and baby to get comfortable with your new, unique breastfeeding routine.

Most breastfeeding sessions are set for at least an hour and a half for most appointments. If you need to cancel, please remember that there are mommies and babies that would love the support to get a great start on breastfeeding. Please try to give us 24-48 hours notice if you are unable to keep your appointment. We will be able to reschedule you for a time that best fits you and your family.
